What is dysmenorrhea?

Dysmenorrhea is pain and cramping in the lower abdomen that women experience during menstruation. These sensations may also spread to the lower back and legs. The pain can range from mild discomfort to a level that interferes with daily life.

Types of dysmenorrhea

1. Primary dysmenorrhea
This type is the most common. It occurs in teenagers after the onset of menstruation and is not associated with any underlying disease. The pain is caused by uterine contractions triggered by the release of prostaglandins.

2. Secondary dysmenorrhea
This type of pain is caused by underlying conditions of the pelvic organs, such as endometriosis, fibroids, or inflammatory processes. It usually appears in women over the age of 20–30.

Symptoms

  • Pain and cramps in the lower abdomen
  • Pain radiating to the lower back and legs
  • Nausea and vomiting
  • Fatigue and weakness
  • Headaches or dizziness
  • Diarrhea or constipation

Causes of dysmenorrhea

During menstruation, the uterus contracts to shed the endometrium. These contractions can sometimes cause pain, especially when prostaglandin levels are elevated in the body. If a woman has pelvic organ diseases, the pain may be more severe.

Treatment and prevention

To manage dysmenorrhea and reduce pain, the following methods can be used:

1. Medications

  • Pain relievers (e.g., ibuprofen or paracetamol)
  • Hormonal medications to regulate the menstrual cycle

2. Heat therapy

A heating pad applied to the abdominal area helps relax muscles and reduce pain.

3. Lifestyle changes

  • Regular physical exercise
  • Healthy diet and adequate water intake
  • Stress management (e.g., yoga or meditation)

4. Medical consultation

In case of severe pain or suspicion of pelvic organ diseases, it is necessary to consult a gynecologist.

What is considered a menstrual cycle disorder?
A menstrual cycle disorder is defined as any deviation from the norm in the frequency, duration, or amount of menstruation. For example, periods that are too infrequent or too frequent, too heavy or too light, or the absence of menstruation for more than three months in women of reproductive age.
Should I see a doctor if my cycle is irregular?
Yes, especially if the disorder occurred suddenly, is accompanied by severe pain, heavy bleeding, a missed period for more than 3 months, or the appearance of other symptoms (acne, facial hair growth, sudden weight loss/gain). A doctor can help identify the cause and prescribe treatment.
What tests help to determine the cause of menstrual irregularities?
Typically, a complete blood count, hormonal tests (FSH, LH, prolactin, estradiol, testosterone, TSH), an ultrasound of the pelvic organs and, if necessary, additional examinations to identify endocrine or gynecological problems are prescribed.
Does age affect the menstrual cycle?
Yes. Adolescents may have irregular cycles for the first 2–3 years after menstruation begins. Before menopause (45–55 years of age), cycle irregularities are also common due to decreased hormone levels.
What causes irregular periods?
The causes can be varied: hormonal imbalance, stress, sudden weight changes, physical overexertion, thyroid disease, polycystic ovary syndrome, pelvic inflammatory disease, or taking certain medications.
How to normalize the menstrual cycle?
Treatment options depend on the underlying cause. Dietary adjustments, stress reduction, moderate exercise, and weight management are often helpful. In some cases, a doctor may prescribe hormone therapy or other medications.
How to distinguish a delay in menstruation from amenorrhea?
A missed period is the absence of a menstrual period for several days or weeks. Amenorrhea is the complete absence of menstruation for more than 3 months in women of reproductive age or more than 6 months after stopping breastfeeding.
Can irregular periods be a sign of pregnancy?
Yes, a missed period is one of the possible signs of pregnancy. If you suspect you are pregnant, it is recommended to take a test and consult a doctor.
